Q-1   What Is Liposuction?

A-      Liposuction is a cosmetic surgery operation that helps sculpt the body by removing fat deposits from specific areas like the abdomen, hips, buttocks, thighs, knees, upper arms, under chin, cheeks, and neck. The fat is usually removed through a cannula (a hollow tube) and aspirator (a suction device). Liposuction is not a substitute for dieting and exercise, but it can remove stubborn areas of fat that do not respond to traditional weight loss methods.

Q-2     On what areas of the body can Liposuction be done?

A-      Common locations for liposuction include the under chin, cheeks, neck, upper arms, abdomen, lower back, buttocks, hips, thighs, knees, calves, and ankles.

Q-3     What areas of the face and body are treatable with liposuction?

A-        Liposuction works best for areas of the face and body with stubborn fat deposits resistant to overall weight loss, such as the infamous “love handles.” The thighs, abdomen, buttocks, flanks, hips, upper arms, chin, jowels, and neck generally respond well. Men and women may also choose liposuction to reduce breast size (Gynocomastia), a procedure that results in less scarring than other types of plastic surgery. Liposuction can be performed consecutively on many different sites; in fact, many patients choose to have several body parts treated during the same day.

Q-4   Who are good candidates liposuction surgery?

A-      These procedures are for anyone who cannot obtain the trim and properly contoured look with diet and exercise, possess good skin elasticity and are not looking to lose large volumes of weight. Patients who exceed their body weight by 30%-50% may only be candidates for limited fat removal because of safety concerns and should undergo these procedures in two or more stages. Patients who exceed their ideal weight well over 50% should seek other remedies for weight loss as an initial step and consult a physician experienced in weight management.

Q-5   Is liposculpture right for me?

A-      If you’re thinking about liposuction it is essential that you have realistic expectations about what the procedure can do for you. It’s important to understand that, while liposuction can enhance your appearance and self-confidence, it won’t necessarily change your looks to match your ideal or cause other people to treat you differently. So before you decide to have surgery, please think carefully about your expectations and do not hesitate to contact your plastic surgeon.

The best candidates for liposuction are normal-weight people with firm, elastic skin who have pockets of excess fat in certain areas. You should be physically healthy. Your age is not a major consideration; however, older patients may have diminished skin elasticity and may not achieve the same results as a younger patient with tighter skin.

Liposuction carries greater risk for individuals with medical problems such as diabetes, significant heart or lung disease, poor blood circulation, or those who have recently had surgery near the area to be contoured.

Q-6     What do I need to know before undergoing Liposuction?

Q-7     I know there are different lipo procedures… But briefly, how is liposuction performed?

A-      In general, liposuction procedures use small incisions in selected areas and with a high vacuum suction device attached to a small, stainless steel instrument called a cannula, remove fat cells. After the fat is removed, sutures (stitches) will be used to close the small incisions, followed by the application of a dressing and compression garment (girdle) to minimize the post-operative swelling.

Q-8     What type of anesthesia is required when having Liposuction?

A-      Various types of anesthesia can be used for liposuction procedures. If a small of amount of fat is being removed from one or two areas local anesthesia with a light sedative to make you feel drowsy and relaxed may be considered.

When treating larger areas (3 or more), general anesthesia is typically used causing you to be asleep during the procedure. Together, you and surgeon will select the type of anesthesia that provides the most safe and effective level of comfort for your surgery. I personally will request general anesthesia even for a small area. I have seeing cases where patients feel some level of discomfort under local anesthesia.

Q-9     What are the main risks and complications for liposuction surgery?

A-      Every surgical procedure has risks and liposuction is no different, but the American Society of Plastic and Reconstructive Surgeons has recognized liposuction as a safe and effective method of removing localized fat areas. The most common risks of liposuction include infection, allergic reactions to anesthesia or to medications, friction burns, organ damage, fluid accumulation, blood clots, excessive fluid loss, and long healing time. The amount of fat being removed can increase the risk of liposuction because more fat will take longer to operate on.

Q-10   What can I expect after my Liposuction procedure?

A-      Don’t expect to feel or to look great following your surgery. Even though the newer techniques are believed to reduce post-operative discomforts, you may still experience some pain, burning, swelling, bleeding and temporary numbness that could last as long as one year. You will be wearing a snug elastic garment over the treated areas to control swelling and bruising immediately following your surgery. You may also experience some drainage from the incisions sites for two to five days following surgery. Pain can be controlled with medicates prescribed by your plastic surgeon, though you still may feel stiff and sore for several days.

Q-11   Will I Have Scars From Liposuction?

A-      Scars are a normal occurrence when incisions have been made. Liposuction is no different since it involves incisions, however the surgeon will carefully treat and place the small incisions in areas that are better concealed or in creases. Over time the scars from liposuction will begin to fade until they are hardly noticeable. There will always be a hint of a scar remaining after liposuction.

A-        Liposuction is considered a cosmetic surgical procedure and will not be covered by insurance. Typically, liposuction will cost an individual between USD $4,000 for 2 small areas and as much as USD $10,000 for 5 areas, but your liposuction surgeon will have a more specific idea of the liposuction cost after a consultation. On average, liposuction in the U.S. costs $2,000 per body area treated. Q-13   Will the fat return after a liposuction?

A-        If the liposuction patient gains weight the entire body will distribute the fat because the treated area no longer has the same capacity to store additional fat because a large portion of the fat cells were removed.

Q-16   How will I be evaluated for liposuction?

A-        The liposuction candidate will go to a consultation so a board certified plastic surgeon can evaluate if liposuction will benefit them. The surgeon will want to discuss the interested liposuction candidate’s medical history, in addition to your physical traits and goals they wish to attain through liposuction. The more honest the liposuction candidate is with the surgeon, the more likely the individual will be happy with their results because the surgeon can inform the candidate if the desired liposuction results are attainable.

Q-17   What are some of the most common benefits of Liposuction?

A-        Liposuction is the most commonly performed plastic surgery in the United States. This technique is used to reshape the body by permanently removing localized fat deposits. A newly trim body can provide a psychological lift and an extra incentive to exercise regularly to maintain fitness. Many patients even find physical activity more pleasurable after shedding extra fat, and they also find that clothes tend to fit them better.

Q-18   Does the removal of fat tighten the skin as well?

A-      Not exactly. In fact, some fat deposits may be unforgiving to major fat removal and lead to more relaxation of the skin. Many new methods are being investigated in order to test their effect on skin contraction and tightening. Ultrasonic liposuction has been suggested as a stimulus to skin contraction. Some cosmetic surgeons are recommending the application of laser energy to the liposuction site. Unfortunately, it is the heat generated by these modalities that is believed to be the stimulus for the contraction process. If the heat of ultrasonic and laser energy combined with the rigors of the suction pressure is applied too aggressively near the skin, it can lead to serious skin injury. On the other hand, the use of the microcannulaes at less than 3.0 mm in diameter, if applied near the skin in selected areas, can help achieve contraction as the fat is removed with little risk.

Q-19   Can Liposuction/Liposculpture be used instead of going on a diet?

A-      Liposuction/Liposculpture is not a procedure for loosing weight, but is used for remodeling the body, eliminating accumulations of fat. The fat does not weigh very much and after a considerable aspiration you only lose a few kilograms.

Q-20   Is it better to go on a diet before having Liposuction/Liposculpture?

A-        It is useless to go on an extenuating diet before having Liposuction, to then, perhaps, put on weight in the following months. We recommend that our clients have the surgery when they think they have reached the weight they can maintain. In most cases, however, after Liposuction, a series of psychological mechanisms are set in motion which lead clients to be more careful about the way they eat and in taking care of their bodies, so that in the months after surgery, they often experience weight loss and an improvement in their physical condition. These conditions boost the effect of the modeling and firming up of the tissue of the superficial Liposculpture giving a result which is often greater than expectations.

Q-21 How much weight can you lose with Liposuction/Liposculpture?

A-      The scope of Liposuction (Lipoplasty) is not the loss of weight but the remodeling of the body. In the US, It is generally carried out on two well defined areas and not on the entire body. In Countries like Colombia where plastic surgeons are not so restricted by insurance companies, liposuction of the entire body is a common practice.

Q-22 What is the difference between liposuction and liposculpture?

A-      Liposuction is the removal of medium to large amounts of contour fat to reduce the peaks and mounds that portray a heavy and sloppy appearance. These include areas such as below the chin, upper arms, outer breasts, lower abdomen, flanks, hips, and thighs. Liposculpture is the removal of small to medium amounts of fat in rather normal to full areas to sharpen features, accentuate the muscles beneath and create a more cut appearance in the neck, lower cheeks, upper abdomen, outer buttocks, calves, and ankles.

Q-23  Can Mesotherapy eliminate cushions of fat?

A-      Mesotherapy does not usually have any effect on cushions of fat. This type of treatment is none other than a way to convey thropic-effect medicines to fatty tissue. It may also have some effect on (lipodstrophy) but the therapy has to be followed with perseverance (it is never definitive), having it out together with a diet. I personally do not recommend it. For some reason, Mesotherapy changes the texture of the fat deposits making its removal through liposuction a much harder task.

Q-24   Many argue that a local anaesthetic is safer than a general (or total) anaesthetic. Why?

A-      Local anaesthetic is safe if the dosage is not excessive. When the area to aspirate is extensive, the high doses of local anaesthetic required are toxic and may cause serious complications. In these cases, a general anaesthetic (by inhalation) guarantees the best results, reducing risks to a minimum. In any case now, after a few hours, in most cases, the client can return home.

Q-25   On what basis is the decision made to operate under local or general anaesthetic?

A-        The decision depends mainly on the number of areas to be operated on, the amount of fat to be removed from each single area and on the weight of the client. The dose of anaesthetic that could be toxic varies, therefore, according to the case. Most procedures (of a small or medium entity) can, nevertheless, be performed in day hospital under local anaesthetic with sedation. If the fat to be removed is over 2.5 liters, the advisability of operating under general anaesthetic will be considered.

Q-26  Some argue that to obtain good results, Liposuction/Liposculpture should be performed under local anaesthetic with the client in a standing position so that the fatty cells to be removed can be seen better. Is this true?

A-      The cosmetic surgeons in most clinics do not agree. With Liposuction/Liposculpture, it is not always possible to keep a client standing up: blood pressure tends to go down and they could faint. The surgeon must evaluate this. Furthermore, with advanced techniques like superficial Liposculpture, after the fat has been removed, if you are still on your feet, the skin appears to be sagging. The result looks terrible and completely different from that which will be the final outcome after the final modeling and compressive medication carried out by the surgeon.

Q-27   What is a local anaesthetic with sedation?

A-      So-called sedation involves the intravenous administration of drugs which diminish or nullify the discomfort related to surgery. It involves substances that are anxiolytic (for reducing tension), hypnotic (for making you sleepy), analgesic (for killing pain). Finally, some substances induce amnesia (you do not remember almost anything about the surgery). If the client so wishes, it is, nonetheless, possible not to have any sedation.

Q-28 What is the difference between Liposuction, Liposculpture and Lipoaspiration?

A-      Liposuction and Liposculpture are quasi synonyms. Some plastic surgeons refer to Liposculpture when the fat removed its use to refill sunken areas (Lipofilling or Lipostructure), to obtain a global remodeling of the areas to be treated.

Q-29   What are the possibilities of having Liposculpture on the calves and ankles?

A-      Lipoaspiration of the calves and ankles is possible and, if the conditions are right, you can achieve good results; if the volume of the calves is caused by muscles and tendons or bones, very little can be done. Water retention is not a contraindication but, after the procedure, it may continue and, therefore, affect the validity of the end result. In this region, post-operative swelling lasts much longer than in the other areas (6 months to a year). For this reason, it is advisable to have the surgery in the winter months and then have Lymphodrainage treatments.

Q-30  If you have problems with your circulation, can you still have Liposuction/Liposculpture?

A-      Yes, the surgery can be performed in most cases with a good outcome, both aesthetically and physically. The type of surgery must, however, be analyzed during the course of the consultation.

Q-31  Is it necessary to stop taking the pill before Liposuction?

A-      It is advisable but not obligatory. Women who take the pill need to take a course of anticoagulant drugs (deriving from heparin), as preventive medicine, both before and after surgery.

Q-32   What is the degree of risk with Liposuction/Liposculpture? Is it true that they are now considerably less?

A-      The risks of Liposuction/Liposculpture in healthy people are those common to all surgical procedures (reactions to the anesthetic, infections, etc.) but complications are very rare. It is, however, necessary to operate with the proper precautions: suitable clinics, assistance with the anesthetic even in procedures performed under local anesthetic, the use of a general anesthetic in more extensive Liposuctions (giving too much anesthetic risks being toxic). The swelling technique and the use of fine cannulas guarantee the best results with very little bleeding.

Q-33   Isn’t there the risk that, where the fat has been removed, the excess skin will sag?

A-      For this reason, the Cosmetic Surgery specialists in many clinics prefer Liposculpture, that is, superficial Liposuction which, apart from reducing the cushion of fat, increases the skin tone. Moreover, the surgeon has to take into account, at least generally, whether the skin will be elastic enough to adapt to its new condition. Many plastic surgeons prefer thick cannulas because they can remove the fat with less effort. I personally see better results with fine cannulas and the bleeding its in most cases minimum.

Q-34   What is Lymphodrainage?

A-      Lymphodrainage (LPG) is a special massage which aids in the re-absorption of pre- and post-operative swelling thereby speeding up recovery. It can also be performed, with a great many advantages, with the LPG – Endermologie method. It must be done by expert personnel who have taken special courses.

Q-35   After Liposuction/Liposculpture, is it indispensable to have Lymphodrainage?

A-      Lymphodrainage speeds up recovery and the swelling goes down much sooner. It is not indispensable, but it is, nonetheless, fundamental. I strongly recommend lymphatic drainage for all types of liposuctions and Abdominoplasty procedures (tummy tuck). It really makes a big difference.

Q-36   How long after the liposuction procedure can I return to the gym?

A-      The pain is usually bearable and can be alleviated with analgesics (pain killers). It is subjective and varies according to the individual and the extent of the surgery. You can start walking immediately and resume sporting activities after 3 weeks according to your case and the kind of sport involved.

Q-37   How long after giving birth can you undergo Cosmetic Surgery?

A-      The time that should pass after a birth before undergoing Cosmetic Surgery varies and depends on both the kind of surgery and the general condition of the client which may be different according to the case. With regard to Liposuction, the procedure can be performed after 3 months if you are not breast feeding.

Q-38  In what period of the year is it best to have Liposculpture?

A-      Even though the surgery can be performed at any time during the year, the cold months are to be preferred mainly because after the procedure the client should not be exposed to the sun for at least a month and because, when it is hot, it is uncomfortable to wear a compression garment. If these factors cause no problems, Liposuction/Liposculpture can also be performed during the summer.

FACIAL LIPOSUCTION

Q-39   What is a facial liposuction?

A-        Facial liposuction removes fatty deposits from specific areas of the face such as the under chin, neck, and jowls. Facial liposuction rids patients of these excess fat deposits that make them look and feel older. A safe procedure with dramatic results, facial liposuction has helped thousands of people look and feel younger and more attractive.

Q-40   How is face, neck or under chin liposuction performed?

A-      Facial liposuction is most often performed with a micro-cannula using a tumescent technique that involves injecting fluid into the targeted area while suctioning the fat out. In some cases, liposuction of the chin, neck, and jowls, as well as other facial liposuction procedures, can have better results in these problem areas than other surgical techniques. Facial liposuction can also be more desirable because it minimizes scarring when compared to other surgical techniques. Facial liposuction is usually performed on or below the chin and jaw line to reduce the appearance of sagging skin in the neck, a double-chin, or hanging jowls.

Q-41   Who is a Good Candidate for Facial Liposuction?

A-      The best candidate for facial liposuction is someone who wishes to remove bothersome pockets of fat but is within 30 percent of their ideal body weight. Age is not a major consideration in facial liposuction procedures, but those with more elastic qualities in their skin can expect to experience better results. For those who do not have elastic skin, other procedures such as a face lift can tighten the skin after fat has been removed using liposuction techniques. The best way to determine if you are a candidate for chin, neck, jowl, or any other type of facial liposculture procedure is to speak with an experienced cosmetic surgeon.

Q-42   What is the recovery time for facial liposuction?

A-      Recovery from facial liposuction is a relatively short process. Patients who have their under chin, jowls, or neck treated with liposuction can expect to experience mild discomfort with some bruising or swelling. This should subside in about seven to ten days. Patients may require a few stitches to close the opening made by the cannula (the tube used to break up and remove fatty tissue). Most facial liposuction patients return to normal activities within one week and experience the full benefits of surgery within a three to six month period.

BODY CONTOUR LIPOSUCTION

Q-43   Liposuction of the Abdomen. How effective is it?

A-      Liposuction is very effective for shaping the abdominal area in men and women. The lower abdomen in particular tends to be resistant to diet and exercise, making even the most fit and active individuals insecure about their appearance. Pregnancy and cesarean births can have a disfiguring effect on the lower abdomen, which diet and exercise cannot correct. Most abdominal fat is located directly beneath the skin (subcutaneous), and can easily be removed through liposuction procedures. However, fat that is located deep in the abdomen (between the intestines- most common in men) cannot be removed with liposuction because of the increased risk of severe complications.

Q-44   Liposuction of the Flanks. Where my flanks are located?

A-      Flanks also known as love handles are located just above the belt line on the sides and back. Liposuction can greatly improve the appearance of flanks and reduce the amount of fat that bulges over the belt.

Q-45   Liposuction of the Hips. Is it recommended for men?

A-      Liposuction of the hips is generally successful in creating a more contoured figure. This type of liposuction is usually performed on female hips as men are not as susceptible to retaining fat deposits in this area. The procedure is usually performed under general anesthesia. Patients who undergo liposuction to improve the appearance of the hips may require additional procedures to attain the desired contour.

Q-46   Liposuction of the Buttocks.  Can buttock liposuction be performed with buttock lift?

A-      Since the normal aging process affects the firmness and elasticity of the skin, older patients may not achieve quite the same results as younger patients. Buttocks liposuction is often performed in conjunction with a buttocks lift or with thigh liposuction. It is important to remember when considering liposuction that shaping the buttocks is more important than reducing the size. Excess removal of fatty tissue in the buttocks can result in an asymmetrical, lumpy, or sagging appearance. The buttocks serve the functional purpose of acting as a soft cushion on which to sit, and are also important in the overall aesthetic appearance of the body. Surgeons should approach liposuction of the buttocks with the subtle touch of an artist, and should always use a micro-cannula, not more than three millimeters (1/8 inch) in diameter.

Q-47   What are the other parts of the body treatable with liposuction? 

A-      Liposuction of the Thighs

         Liposuction of the entire thigh (circumferential liposuction) is not recommended due to an increase in swelling and lengthened recovery time. Liposuction of the thighs should be performed in a series of surgeries spaced about one month apart. Focusing the liposuction on one targeted area of the hips allows the lymphatic tissue to drain excess fluids quickly and evenly throughout the thigh, yielding more desirable results in a shorter amount of time. The above is common practice in the US, in Colombia plastic surgeons in most cases will do the entire thigh.

            Liposuction of the Arms

         Liposuction of the arms has a high rate of patient satisfaction. Liposuction of the arms yields a slimmer appearance that can give the rest of the body a more contoured look.

            Liposuction of the Tummy

         Liposuction of the tummy is a very common procedure for both men and women. It is especially useful in patients who have already lost excess weight through diet and exercise. Liposuction can, in many cases, be the only successful method of eliminating stubborn fat deposits that remain in the tummy and flank region.

            Liposuction of the Breasts

         Enlarged breasts in women can cause back pain, neck pain, headaches, and poor self esteem. Liposuction is a more effective and less invasive method of breast reduction than other forms of breast reduction surgery. Using liposuction to remove excess fat has a natural lifting effect on the breast because it results in less volume weighing it down. I personally have not seeing very good results of breast liposuction

            Male Breast Liposuction

         Liposuction can also be used as a form of breast reduction in men, but several important factors must be considered first. The surgeon must determine whether the excess tissue in the breast is fatty or glandular. If the breast is primarily fatty tissue, then liposuction is an excellent method for reducing the size and improving the appearance of the chest.

         If a man’s breasts are enlarged with glandular tissue, liposuction is not an option and other hormonal or medication therapies should be pursued. If the breasts are asymmetrical or oddly shaped, the patient should be examined for possible tumors prior to any surgical intervention. A mammogram is the most effective method for detecting tumors and the possible presence of male breast cancer.

Abdominal Liposuction (suction assisted lipectomy)

Q-48   What is an Abdominal Liposuction?

A-        One of the most common of all cosmetic procedures, abdominal liposuction is a technique for re-contouring the body by permanently removing fat deposits from the abdomen. The tendency to accumulate fat in the abdomen ( stomach area )may be inherited, and this fat is often especially resistant to diet and exercise. Many patients become discouraged at having lost unwanted pounds without seeing a flatter abdomen. A common complaint is that exercise is just “making my stomach flatter”, or after months of exercise and overall improvement there has been no change in the waistline. Simply put the overall proportions of the body may be unbalanced, or the abdomen may simply have retained areas of fatty tissue that cause it to protrude. In these cases, liposuction can successfully trim excess fat in the abdomen for a flatter “stomach ” or a thinner waistline, thus achieving a more slender, balanced shape.

Q-50   Can Liposuction of the abdomen endanger a pregnancy in any way?

A-      No. There are no problems in being pregnant after a Liposuction of the abdomen. The small scars are final but, if treated properly (correct suturing, massage, lymphatic drainage), they are usually not very evident

A-      Abdominal liposuction is often performed in conjunction with a tummy tuck, or with liposuction to other parts of the body.

Q-54   Will I need to stay in a hospital?

A-        If the amount of fat to be removed is fairly small, your surgery can usually be performed on an outpatient basis, either in your doctor’s office or in an outpatient surgery clinic. If you are having large amounts of fat removed, or are undergoing several procedures at once, you may need to spend the night in the hospital so that medical staff can monitor your initial recovery. The procedure may be performed under local anesthesia with light sedation, local anesthesia and deep sedation, or general anesthesia.          

Q-55   How long do the results last for abdominal lipoplasty (liposuction)?

A-      Because a portion of the fat cells are permanently removed by liposuction, so you will not tend to regain weight in that area. It is important to note, however, that abdominal liposuction does not guarantee that you won’t gain weight or fat deposits in other areas of your body. Be careful, since fat cells have been removed from the treatment site but not from surround areas, this weight gain can appear uneven. It is important to maintain your weight and contour with exercise and proper diet.

Q-57   What will happen at my initial consultation?

A-        During the consultation, you’ll be able to talk to your doctor about the changes you would like to make in your appearance. The plastic surgeon will explain the different options available to you, the procedure itself, its risks and limitations, and the costs. Q-98   How do I prepare for Tumescent Liposuction surgery?

A-        Tumescent Liposuction preparation requires avoiding medicines that can cause bleeding. Aspirin, Ibuprofen, and many arthritis medications interfere with platelet function and must be stopped 2 weeks before surgery. Many cold, sinus, and other medications contain these drugs and must also be stopped. Vitamin E interferes with the liver making blood clotting factors from Vitamin K. Stop taking Vitamin E at least 3-4 weeks before surgery. Cigarette smoking can hinder wound healing, so please stop at least 3 weeks before your operation.

         To avoid ingrown hairs, don’t shave or remove hair near the regions to be sculpted the day of surgery. You will have to not eat after midnight so you can arrive with an empty stomach. You will be able to take your medications with a small sip of water that morning. Check with yours cosmetic surgeon about any medications that you will be taking. Wear loose fitting clothing that opens in the front or pulls over easily. This will make it easier to get dressed over your pressure garment and dressings.

Q-99   How is the recovery time for Tumescent Liposuction?

A-        Tumescent Liposuction leaves local anesthetic solution in the tissues resulting in much greater comfort after surgery than the older techniques. This prolonged relief can be of value in cases done under general anesthesia or sedation. Once this wears off, most patients experience some discomfort that is usually easily managed with some oral medication for a few days.

         You may weigh more, about the same, or less right after surgery. Some of this is from the extra fluid used during the tumescent technique that you will urinate over the next day or so. Some weight will be from the fluid of tissue swelling. How much and where depends on what was done. Since there is less bruising and bleeding from Tumescent Liposuction, there is less blood to be absorbed and irritate tissues. The lack of this irritation means that the swelling goes down much faster.

         After sedation or general anesthesia, eat lightly at first to avoid an upset stomach. Drinking some extra liquids will help your body adjust its fluid balance. Since there is a lot of fluid injected during Tumescent Liposuction, some does leak out of the tiny wounds after surgery. This is normal and should end within a week. Using feminine pads or dressings to catch this liquid is an effective way to deal with this minor inconvenience. You can resume showering by the second day after surgery.

         Tumescent Liposuction results in a rapid improvement of your shape. Most of the swelling goes down within 10 days but part of the result is from the contraction of tissue that has been treated. Although our  patients see a dramatic improvement within 7-14 days, it will take 3 to 4 months to see most of the changes in your contour.

Q-100 When can I resume my work and activities?

A-      You will need to limit your activity for the first couple of days – no strenuous exercise, lifting or bending over. Avoid any type of aerobic exercise (any activity that raises your pulse above 90) for at least three weeks. That much work may increase your swelling. Many are able to return to work almost immediately. It depends on what you do and the activity level of your job. Resumption of activities also depends on the extent of your Tumescent Liposuction, your health, and how you heal. In general the discomfort lessens significantly in 2-4 days. Stabilizing your tissues in a special girdle (garment) increases your comfort and ability to get around.

Q-101             What garment do I wear pos Tumescent Liposuction?

A-      Stabilizing your tissues in a special garment increases your comfort and ability to do things. Your tissues will be more comfortable after Tumescent Liposuction when they are not jiggling around. Compression also helps control swelling and bruising. Our patients wear this for about six weeks full time and then part time for 2 more weeks. Different styles are available for the various zones treated by the Tumescent Liposuction.             Possible Liposuction Problems

         With any type of cosmetic surgery, including liposuction, complications may arise, but they are rare. Some dangers of liposuction include infection, a reaction to the anesthesia, blood clotting, and fluid loss. Less severe liposuction risks include bruises and scars, a numbing of the skin, and moderate problems such as changes in skin pigmentation. Liposuction complications may include:

• Infection
• Extended healing time
• Allergic reaction to medication or anesthesia
• Fat or blood clots – clots can migrate to the lungs and lead to death
• Excessive fluid loss – fluid loss can lead to shock and, in some cases, death

            Dangers of Excessive Liposuction

         The most common dangers of liposuction include risks associated with removing too much fat from targeted areas at once, as well as having too much liposuction performed in a single day. Excessive liposuction can cause problems including dents, lumps, and sagging skin. In order to minimize surgical complications and the side effects of overexposure to anesthesia, patients should schedule multiple liposuction procedures at least several days apart.

            Minor Complications

It is extremely important to choose a cosmetic surgeon who will clearly and thoroughly explain potential liposuction side effects and risks. He or she should also have proper and extensive training, along with state-of-the-art facilities and a good standing in the field.

         Severe complications can be minimized by disclosing your entire medical history to your doctor and discussing all prescription and over-the-counter medications you take on a regular basis. Patients should be good candidates for liposuction and fully understand what is involved in this procedure.

         To maximize the success of liposuction surgery and minimize the risk of complications, always follow your cosmetic surgeon’s instructions for surgical preparation and postoperative care. Q-108             Ultrasonic Assisted Liposuction. What is it?

A-      Ultrasonic liposuction, or UAL, is a procedure that uses high-pitched sound waves to liquefy fat in a specific area before it is removed. UAL is a type of liposuction that is primarily used for fat removal in hard-to-treat areas such as the chin, neck, cheeks, knees, calves, and ankles. It usually serves as an addition to tumescent liposuction for patients who require more precise body contouring.

Q-109             How do I know whether or not I am a candidate for ultrasonic liposculpture?

A-      Traditional tumescent liposuction has been very effective in young patients with mild to moderate amounts of fat. Ultrasonic liposuction expanded the number of patients suitable for body sculpting. This includes patients with larger amounts of fat and less elastic skin tone. It also allows us to treat areas not easily or effectively treated in the past. This would include fibrous areas such as the back, male breast and upper abdomen. A physical examination is required in order to verify whether you are a candidate for this procedure.

Q-110             What medical conditions might exclude me from being a candidate?

A-      A patient needs to be in good medical condition in order to undergo this or other elective procedures. A patient should have the ability to heal incisions and adequately fight infections, and no medical contraindications such as a heart or lung disease. Heavier patients are now candidates for this type of procedure; however, obese patients with medical contraindications are not candidates. Patients with excessive amounts of redundant skin are not candidates for this procedure.

A-      What makes this so unique is the ability to gently liquify or emulsify the fat without injuring the adjacent structures. This is a non-mechanical method of dissolving the fat that also appears to tighten the skin. There has been less blood loss, less bruising and decreased tissue trauma observed. Ultrasonic liposuction allows us to also removed fat in areas that were preciously unable to have liposuction such as the back, upper abdomen and male breast.

A-      Ultrasonic liposuction, (Ultrasound Assisted Lipoplasty) or “Ultrasound Assisted Liposuction” (U.A.L.) is a technique of liposuction where the fatty tissue is removed with the assistance of sound waves which are beyond the range of human hearing. Because the sound waves are doing the work the surgeon is able to more precisely guide the cannula. This increases safety. The ultrasonic energy is targeted to fat cells and creates less damage to other tissues such as nerves, blood vessels and muscles.

A-      Ultrasonic liposuction can be used in all areas that traditional liposuction treats, but it works especially well on areas that require more precision. The stomach, midsection or “love handles,” male breast tissue, chin, and neck all respond well to UAL, as do areas that have been previously treated with liposuction but are in need of further contouring.

A-      The ultrasonic liposuction equipment is highly sophisticated. The fat is dissolved by the ultrasonic waves that are emitted from the end of a cannula. This liquified fat is then gently aspirated away. While endoscopic techniques have been utilized to inspect the areas in which the fat has been removed, it is not necessary on a routine basis. This is because the results are immediately visible, just as when ice or snow melts in the sun.

A-        The surgeon should review his experience with the various methods and analyze your body type. Some surgeons may advise ultrasonic liposuction for large volume extractions, especially in tough areas such as the back, flanks and hips. It was designed principally for the hard to extract deep fat and to reduce surgeon fatigue. External ultrasonic energy can be applied and is recommended by some surgeons before or after the fat is removed by regular liposuction. Ultrasonic liposuction must ordinarily be combined with traditional liposuction when deep and surface fat is being removed. This does add to the operating time or to the cost of the surgery and may offer no advantage for most surgeons. For gynecomastia (large breasts in the male), some experts advocate the sole use of ultrasonic liposuction but it should not be used aggressively near the nipple skin.

A-      VASER® liposuction emulsifies fat prior to removal, facilitating the liposuction process and reducing pain and tissue damage. LipoSelection® uses an ultrasound probe to break up fat before it is removed. This type of liposuction reduces pain and results in a faster healing time.

            The Vaser® Liposuction Procedure

         LipoSelection®, developed by VASER®, is an ultrasonic (UAL) liposuction procedure, meaning it uses ultrasonic energy as a means of fat removal. The ultrasound technology breaks up and emulsifies the fat that is to be removed. Unlike other fat removal methods, LipoSelection® leaves the surrounding nerves, blood vessels, and tissue virtually unharmed. After a local anesthetic is applied, the surgeon will fill the area to be treated with a saline wetting solution to numb the site and shrink blood vessels. This helps minimize post-surgical bruising and swelling. A small probe emitting high frequency sound energy is used to break up fat on contact with minimal damage to surrounding tissues. Fat may then be easily extracted with liposuction tubes.

A-               The risks of VASER® liposuction are very similar to the liposuction risks associated with any other method. Serious liposuction complications are quite rare. Fat clots or blood clots may form, and there is a chance these will migrate to the lungs and cause serious illness. The fluid loss that results from liposuction will be anticipated by a qualified doctor, as it can cause low blood pressure, friction burns, or fluid buildup. The LipoSelection® probe’s ultrasound energy may injure the skin or deeper tissues, although in most cases the benefits of the ultrasound system make it safer than traditional liposuction. If a doctor inexperienced with the LipoSelection® procedure applies too much fluid, the wetting solution may collect in the lungs. Power Assisted Liposuction Procedure

Q-125             What is PAL (power assisted liposuction) ?

A-      PAL uses a powerized cannula which moves back and forth through the fat tissue in a rapid motion. PAL is not ultrasonic. No heat is generated to harm healthy tissues. With PAL, less force is required to remove unwanted fat, especially in areas of more fibrous tissue. That makes the procedure easier on both patients and surgeons.

Q-126             Who can be treated with power assisted liposuction?

A-      Both men and women can benefit from Power Assisted Liposculpture. Weight and age restrictions have been lifted. Most patients who have localized fatty deposits, are within thirty pounds of their ideal weight, and have been stable at that weight for years.

Q-127             Who are the best candidates for PAL?

A-      The best candidates:

         • Have firm, elastic skin and are in good health.
         • Have tried diet, exercise and weight loss programs.
         • Have disproportionate fat deposits in their body.
         • Have realistic expectations.
         • Don’t expect liposuction to change their lives dramatically.

Q-128             Which areas of the body are most frequently treated with PAL?

A-      The most frequent areas treated by PAL in men are the “love handles,” breasts, abdomen and neckline. In women they include: thighs, hips, buttocks (saddle bags), stomach, knees, lower leg, ankles, jowls, and chins. Some women elect to have their fat recycled. Through a procedure called lipo-augmentation, their fat is transferred to other parts of their body, like their lips, for example.

Q-129             Is the fat reduction permanent?

A-      Yes. It seems that after puberty we do not create new fat cells. When you gain weight, you expand the fat cells that you have. Power Assisted Lipo-sculpture permanently removes fat cells to improve your body’s contour. Should you gain weight after your PAL procedure, it goes to the fat cells that remain in your body. The localized fatty deposits that existed prior to liposculpture can no longer balloon up out of proportion to the surrounding areas.

Q-130             Does Power Assisted Liposuction remove cellulite?

A-      Although many patients do notice an improvement in the appearance of cellulite after the fat cells are removed, liposuction is not a treatment for cellulite.

Q-131             Will I need a second procedure?

A-      For most patients, only one procedure is necessary. Power liposculpture appears to reduce the number of patients requiring touch-up procedures in the future.

         Cosmetically speaking, successful liposuction is measured in inches rather than pounds. The weight removed is not as important as the improvement in your body’s contour. Most important is the patient’s improved self-image and self-acceptance.

         Dimples, ripples and stretch marks remain unchanged following liposuction. The changes following liposuction are similar to what you would expect if you could lose an equal amount of localized fat through diet and exercise alone. The use of smaller cannulas allows the fibrous connections between your skin and body to remain. These connections will contract over time. As a result, excessive skin folds are so rare that many patients avoid the necessity for surgical skin removal by having excellent results with liposuction alone.

Q-132             What will my recovery be like?

A-      You will wear a snug elastic dressing, girdle, or body stocking over the treated area for the next several weeks. You will go home after the procedure and rest for at least 24 hours, after which you may gradually increase your activity as tolerated. Most patients return to work two days after surgery.
